- New students, enter the full names of each student EXACTLY as you wish for them to appear on the trophy and enter the number of years (including this year) that the student has danced.
- If you danced with us in the past but did not receive a trophy from us LAST year, enter the full names of each student EXACTLY as you wish for them to appear on the trophy and enter the number of years (including this year) that the student has danced.
- If you danced with us last year AND received a correct trophy, please DO NOT fill out this form. We will use last year’s information and update the year.
- If you danced with us last year and received an INcorrect trophy (name misspelled or incorrect years), please DO NOT fill out this form. Please email Amanda with the correct trophy information.
